In front of 87-89 Fort Avenue: A section of the brick sidewalk has completely caved in and collapsed, leaving a deep hole with several loose, displaced bricks. There is a severe uneven trip hazard surrounding an exposed iron utility cap. The rest of the surrounding brick walkway is also badly cracked, shifted, and uneven, creating multiple separate stumble hazards. This is a high-traffic pedestrian walkway right next to residential entry steps, posing a major risk for anyone walking, pushing strollers, or utilizing mobility devices.

This truck just drove over and cracked the sidewalk ramp pad at Jamaica and Woodman, then parked in the adjacent church lot

316 and 318 Warren st side walk. This side walk has a missing utility hole cover. This is a very busy bus stop for the community and older lady almost walk into it just now. The city just did the concrete slab over about 1 week ago but didn't reinstall the utility cover.
