Please close empty tree pit permanently as it is now too close to the new light pole/cell antenna and a new tree will not be replanted. |

Side walk is badly cracked. Cars and Commercial vehicles drive onto it daily. While repairing it may work to add a pole of some sort to discourage it. ( no parking or slow) thank you |

Update 3/15, constituent called to report that nothing has been done about this problem. No longer a light pole at this location, though there is a large orange cone by the hole now. IN ALLEY Pole set up in case 101002636768. However, around the pole is just loose dirt. Constituent requesting the remainder of the hole is filled in with cement to keep the pole in place and cover the dirt. | What is the cause of damage: [Other]

Constituent states that gas construction and snow plows have damaged the sidewalk. The entire sidewalk is cracked, including the constituent's driveway. Across the street, next to the telephone pole, the sidewalk has turned into rubble due to deterioration. Constituent is concerned that the telephone pole may fall over due to the deteriorating sidewalk. | What is the cause of damage: [Construction] What material is the sidewalk made from: [Concrete]

Outside of the handicapped entrance to the building, a barrier/ pole has been knocked down creating a large gap and uneven sidewalk. |

a Stop Sign was knocked down and never put back. We installed it on the light pole but there is a hole where the sign was that needs to be filled. Also that entire length of sidewalk is heaving and is a trip hazard. The sidewalk is the city's but the walkways leading to the sidewalk are the property's responsibility. the issue is with the sidewalk. |
