Description: The constituent is a male age 52 who is homeless and disabled. The constituent is in need of help with resources for food and housing. The constituent also does have a letter from a doctor and a social worker that is speaking on his behalf. The constituent states that he called housing stability to tell him his situation, but the person hung up on them. I gave him some other resources to look over until hes being contacted from these departments. I attached age strong because of the age and hes disabled and needs assitance to file some forms if hes eligible/qualified for services in Age strong department.
