Subject: Updated broken sidewalk Incident Report – Correct Location Information I am submitting this update to my previous incident report regarding my fall on Baker Street. At the time of the original report, I was disoriented and attempted to identify the location using Google Maps, which resulted in an incorrect address being provided. I have since revisited the area and confirmed the correct location. I am attaching photographs showing the damaged/lifted sidewalk at the light pole just before 157 Baker St and previous report attached as well. I am providing this correction to ensure the records accurately reflect the incident location and to help prevent potential injuries to others, as the sidewalk remains in a hazardous condition.
