October 22, 2013 Inspectional Services Department 1010 Massachusetts Ave. Boston, MA 02118 RE: Notice of Violation-3rd Notice Regulation 1.0 Air Quality Demolition/Fire Balconies Building Permit SF284301 Issued 8/27/2013 Dear Sir: Please be advised the demolition work at 341-343 Marlborough St., Boston, MA continues to be conducted in violation of City, State and Federal environmental rules: 1. Debris thrown from upper floors on to roofs. 2. No cover on truck bed used as dumpster. 3. No water being available on site for controlling dust or fire while demolition work continues to be performed (new water lines being installed to 341-343 and demolition work continuing without interruption). 4. Some windows not covered with plastic mesh. 5. Outside areas left in a state of disarray. 6. Dust filtering all over neighborhood, on cars, clogging air filters on AC units, seeping into neighbor’s homes. 7. There method of controlling dust and debris is by using a leaf blower!!! As I have stated before this needs immediate attention; it is a hazard to the neighbors, dangerous and adversely impacts the city environment. The contractor has been advised by several residents and this situation continues. What must we do to correct this? We were also advised that the building owner have plans to remove existing fire balconies used by 339 and 345 Marlborough St. adjacent buildings that are shared with 341-343. This cannot and must not be done for the safety of the residents of 339 and 345 Marlborough St. Any change proposed in these balconies must be discussed with the adjacent owners by ISD before any determinations are made. We ask that ISD immediately address these issues and call with a response as to what is being done to correct the debris removal violations, fines and work stoppages involved, as well as addressing the fire balcony issue. Very truly yours, George M. Locarno | Construction debris: [Wood, lathing and plaster debris] Exact location: [Alley 415, between 341-343 Marlborough Street, Boston, MA]

Clr stating construction work taking place down the street but not in his building 5-6 houses after his there is renovation work. He is upset that bags were dumped in front of his property. Asking for CE to inspect the debris to find any evidence of site responsible and to help coordinate assistance from PWD to remove the bags because trash contractors did not take them. He does not want to be fined for this since it is not even his normal trash collection spot. His trash is placed near Alley. Please inspect for removal assistance. I notified CE to assist regarding above and was told to place into ECM. | Construction debris: [Wood, boxes, etc in 6-7 construction bags] Exact location: [Sidewalk]
